Anthem Blue Cross and Blue Shield Plan F is available to members and their spouses who wish to obtain Medicare eligibility with unlimited drug coverage. The Unlimited Prescription Rider is available to members through the NHCMA and is not available on a direct individual basis. For those who prefer, there is also coverage without the RX Rider.
CIGNA offers two plans. Comprehensive Dental Plan and CIGNA Dental Care (DMO) provide benefits for Type I, Type II and Type III Services. The managed dental plan offers orthodontia coverage and unlimited maximum benefits when using participating dentists.
Metlife's Long Term Care insurance pays expenses up to the daily maximum benefit for: nursing home care, home health care and personal care, assisted living facility, alternate care facility, and skilled care. Daily benefits can range from $50 to $350. The maximum benefit period can range from two years to lifetime and the waiting period can range from 0 days to 365 days depending on plan selection. Coverage is good worldwide, portable and guaranteed renewable. New Haven County doctors and employees receive a 22% premium discount.
Coverage can be designed to meet your individual circumstances and specific needs. It is an individual noncancelable and guaranteed renewable policy. Rates are guaranteed not to change to age 65 and an "own occupation" definition is available. Various benefit durations, elimination periods, inflation protection, partial disability and Social Security offsets can be incorporated. New Haven County doctors and employees receive a 15% premium discount.
The individual disability program under Massachusetts Casualty Insurance Co. has remained stable in its retention of members. Massachusetts Casualty was sold to Centre Solutions Group, a member of Zurich Financial Services Group by Sun Life of Canada last year. No new policies are being sold as a result of this transaction. No change in the terms and conditions of these policies will occur as a result of this transaction. These policies are noncancelable as long as policyholders continue to pay their premiums in accordance with the terms of their policies.
